You can contact the Data Protection Officer on all matters concerning the processing of personal data and the exercise of rights related to data processing.
Purposes of processing and legal basis for processing:
Your personal data provided in this form will be processed by TU Zdrowie for the purpose of verifying if it is possible to include you in your employer’s group insurance coverage, i.e. on the basis of Article 6(1)(f) of the GDPR, where the controller’s legitimate interest is to accomplish the above processing purpose.
Data categories:
Identifying information (first and last name, home address, phone number, email address, PESEL/passport number, employee number, employer name).
Source of data:
If you are a family member of the Primary Insured, your data has been provided to TU ZDROWIE S.A. by the Primary Insured.
Data retention period
Data will be stored until confirmed by your employer, but for no longer than 120 calendar days.
Data recipients:
TU Zdrowie outsources your personal data to your employer for organizational purposes related to accession to a group insurance policy with TU Zdrowie, and outsources your data to entities operating information and communication systems and entities providing legal assistance services, and may share your personal data with state authorities under the provisions of law.
Data subject’s rights:
You have the right:
- to request access to personal data;
- to rectify personal data;
- to request data be deleted; mind that this will prevent further processing to join the group insurance policy;
- to request data processing be limited;
- to object on grounds relating to the data subject’s particular situation to data processing on the basis of a legitimate interest pursued by the controller;
as well as the right to file a complaint with the President of the Office for Personal Data Protection.
Data Requirement Information:
Providing personal data in connection with your wish to be access a group insurance policy is necessary to conclude an insurance contract. TU Zdrowie may not take action to conclude an insurance contract should you not provide personal data.
