TU ZDROWIE carries out health programmes for and on behalf of Nationale-Nederlanden Towarzystwo Ubezpieczeń na Życie S.A. within the scope of medical services covered by the insurance selected by the Insured, as detailed in the insurance terms. 

Medical services are performed only within the duration of insurance cover, are fully coordinated by a Medical Consultant and aim at restoring or maintaining the Insured Person’s health.

In order to begin to avail the services offered by the health programmes, you are required to send a “Start a Health Programme - Application Form” together with a copy of medical records that confirm the occurrence of the event.

We recommend sending “Start a Health Programme - Application Form” online via the Insured Person’s Platform.
