Information clause – video surveillance

The content of the information clause results from the implementation of the information obligation laid down in Article 13 of the Regulation (EU) of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data and repealing Directive 95/46/EC (General Data Protection Regulation) – referred to as the GDPR. 

  1. The data controller is Towarzystwo Ubezpieczeń ZDROWIE S.A. with its registered office in Gdynia, ul. Śląska 17 (hereinafter referred to as TU ZDROWIE S.A.).
  2. TU ZDROWIE S.A. has appointed a Data Protection Officer. You can contact the Officer by sending an email to iod@tuzdrowie.pl or by writing to the address of the controller’s registered office. You can contact the Data Protection Officer in all matters concerning the processing of personal data and the exercise of rights related to data processing.
  3. The processing of data by means of video surveillance is carried out for the purpose of securing property and ensuring the safety of persons on the Company’s premises, pursuant to Article 6(1)(f) of the GDPR, and includes video surveillance of the entrance to the offices and the server room.
  4. The video surveillance recordings are stored for a period of 3 months from the recording or until the end of proceedings in which the recording may constitute evidence.
  5. Your personal data can be transferred only to entities authorized to obtain personal data under applicable law.
  6. You have the right to:
    a) access to your data;
  1. b) to rectify your data;
  2. c) to have your personal data erased;
  3. d) to have the processing limited;
  4. e) to object to the processing of your data;
  5. f) to lodge a complaint with the President of the Office for Personal Data Protection (to the address of the Office for Personal Data Protection, ul. Stawki 2, 00-193 Warsaw).
  1. Your personal data will not be subject to automated decision-making, including profiling.
  2. Personal data will not be transferred to a third country.
  3. Providing personal data in the form of an image is necessary for technical reasons to be on the Company’s premises. On the premises of the Company there are signs indicating that the facility is monitored.
