Information clause regarding the processing of personal data for contact or marketing purposes

You can contact the DPO on all matters concerning the processing of personal data and the exercise of rights related to data processing.

Purposes of processing and legal basis for processing:

1. If you complete a contact form or send us an inquiry in any other form, your data will be processed for the purpose of answering the question or fulfilling the requested contact. The legal basis for the realization of this purpose is Article 6(1)(f) of the GDPR, i.e. the legitimate interest of responding to the completed form or the question asked.
   
2. If you give appropriate consents for TU Zdrowie S.A to direct marketing activities to you through specific communication channels, your personal data will be processed by the Company for direct marketing of our products and services.
   The legal basis for the realization of this purpose is Article 6(1)(f) of the GDPR, i.e. the controller’s legitimate interest in conducting marketing activities in connection with the consents you have given regarding receiving marketing content through specific communication channels.

ID information (first and last name, company name, business phone number, business email address.

Data retention period:

Data processed for contact purposes will be kept by TU Zdrowie for the time necessary to clarify your case and provide a comprehensive response, or for the time necessary to defend and protect claims.
With regard to the processing of data for marketing purposes, TU Zdrowie S.A. will process your data until you withdraw your consent to receive marketing content from us through a specific communication channel, or until you raise an objection

Data recipients:

TU Zdrowie outsources your personal data to entities providing ICT support services to TU Zdrowie; entities providing legal assistance services, marketing services, and may share your personal data with state authorities as required by applicable law.

Data subject rights:

You have the right:

• to request access to personal data;
• to rectification of personal data;
• to request for deletion of data, but this will prevent further processing to join the group insurance policy;
• to request that processing be limited;
• to object on grounds relating to their particular situation to their processing on the basis of a legitimate interest pursued by the controller.

As well as the right to file a complaint with the President of the Office for Personal Data Protection.

Provision of data is not mandatory, but necessary for the purposes of processing described above.
