PREPARATION FOR OCCUPATIONAL MEDICINE EXAMINATIONS

Please be aware that for the examinations you should come rested. Please take your glasses/contact lenses with you. Your driver’s license will be required. Please do not consume any alcohol or stimulants two days prior the examination.

Please provide the sanitary-epidemiological test results for the appointment.

Please provide the pregnancy notes or hospital discharge for the appointment.

For the appointment please bring with you a certificate of treatment completion or medical certificate, that there is no impediment to employment.
Note
In order to undergo the examinations, you will be required to present a referral with UNIMEDI logo, and your ID. Referrals from employers with e-signatures in a non-paper form are accepted, under the condition that prior to the appointment the document is sent to UNIMEDI Occupational Medicine Department and approved by the medical facility that the appointment was made in. Unimedi consultant provides the information every time they present you with the appointment details.

Please bring documents proving your state of health (e.g. a certificate from a diabetologist, neurologist, opthamologist, if you suffer from medical conditions such as diabetes, epilepsy, eye illnesses, etc.)
