The Insured Person can receive TU Zdrowie Assistant’s support in the process of diagnosis and treatment of Lyme disease: get a consultation with a medical practitioner specialising in diagnosing and treating Lyme disease, indicated by the Assistant.

You need to send to TU Zdrowie an application form with medical records, i.e. including lab test results (copies of alarming test results of enzyme-linked immunosorbent assay ELISA for both IgG and IgM class antibodies, optionally results of the Western blot test) through the Insured Person’s Platform or to the email address ubezpieczenia@tuzdrowie.pl
After signing in to the Insured Person’s Platform, the Insured Person needs to fill in an online form "Application to Commence Assistance with Lyme Disease Diagnosis and Treatment" and send it to TU Zdrowie or on www.tuzdrowie.pl the "Application to Commence Assistance with Lyme Disease Diagnosis and Treatment". form is available to download. Having filled it in and signed it, you need to send it to the email address: ubezpieczenia@tuzdrowie.pl together with your test results.
TU ZDROWIE Assistant will contact the Insured Person within 3 working days and will confirm that the assisting process has begun.The TU Zdrowie Assistant may ask for further clarifications and medical documentation. After the process has begun, the TU Zdrowie Assistant provides a phone number and an email address for contact. This way the Insured Person may direct any queries or concerns about the diagnosis.
The TU Zdrowie Assistant in consultation with the Insured person organises the diagnosis process, i.e. the necessary services or indicates the place of their performance.
The TU Zdrowie Assistant supports the Insured Person with information on how to act after being bitten by a tick, in the process of diagnosis and in the ongoing treatment, i.e. taking antibiotics. -
After retiring, the Insured Person has a possibility to remain insured under an individual health insurance with a premium in the amount negotiated by the employer.
In order to do so, you need to complete a form at least 1 month before retiring.

No referrals from primary care practitioners needed to consult specialists for adults and children (consultations with professors included)
Referrals for diagnostic tests accepted from EVERY PRACTITIONER, that also includes doctors from outside the partnership network of medical facilities (e.g. from a National Health Fund GP)
You can choose your favourite health clinic or your favourite doctor.
Information on Lyme disease

- Most commonly you can find it in grass, weeds, bushes on the underside of leaves.
- It doesn’t climb higher than 1-1,5 meters. -
- It looks for a host mostly in the early or late hours of the day, less frequently in the afternoon, when it is hot and sunny.
- It doesn’t like heat, prefers shadowed places. -
- Once it feels a host, the tick falls on the shoes or uncovered legs and climbs its way up to the place where the skin is thin and well supplied with blood.
- You don’t feel it biting you - its saliva contains special anaesthetics.
